Vantage Provider Search Instructions

1. This is the link to a PDF document containing all of our providers. It requires that you have acrobat reader to view the document. If you need acrobat reader, it can be downloaded here.
   
   
2. This is a drop down of all physician specialties. If you are looking for a dermatologist, for example, you can select “dermatology” from the menu to narrow the results to display only dermatologists (along with any other criteria you may have chosen).
   
   
3. This is a dropdown with all of the Vantage Regions listed. Select the region you are in to narrow your results to that region only. If don't know where your region is located, click on the "view regions" link to the right of the dropdown to view a map of our defined regions. This map is a pdf and requires that you have adobe reader to view.
   
   
4. This is a drop down of all the cities with providers in our network. Selecting this option will narrow your results to only the city selected (along with any other criteria you may have chosen).
   
   
5. In the “First/Last Name” field, you can type first and/or last name of a physician. You can also type partial words if you are unsure of the exact spelling. Typing the word “davi”, for example will display names like David, Davidson or Davis.
   
   
6. Click the button to perform the search and display the results.
